Location
Connie Frazier, RD, LDN, CDCES, is an inpatient diabetes navigator with AHN Medicine Institute, specializing in comprehensive diabetes education/management, medical nutrition therapy, and outpatient diabetes care. She received her Bachelor of Science in Dietetics from Seton Hill University in Greenburg, Pennsylvania. She also studied to become a Certified Pump Trainer in order to help aid diabetic patients with the insulin dosage required for each individual. Connie is a certified diabetes care and education specialist, as well as a licensed dietitian. She is affiliated with the Association of Diabetes Care and Education Specialists. She sees patients ages 18 and older and approaches each patient with sincerity and care.
